GMI - List Bill Forms

Even though Global Medical Insurance (GMI) plans are individual and family based plans, in case the employer is paying for several employees (and optionally their families), it is possible for them to pay with a list bill. Please note that it is simply for the convenience of the billing and it is not a group plan.

  Use this form when transferring from an existing medical insurance policy to a Global Medical Insurance plan.
