When you go to the providers within the PPO network, there will be network negotiated fees for eligible expenses.
You can also go out of the network, but there would be no network negotiated fees. Direct billing depends on the provider.

You will have to first pay out of your pocket, and then file for reimbursement of eligible expenses as an out-of-network benefit.
There is no PPO network outside the USA. You are free to visit any provider of your choice. While the insurance company is willing to accept direct billing from providers around the world, it would be up to the provider to do so.
